To add a Survey, select the +Add Survey button on the Surveys page.

The +Add Survey button will display a form to enter details about the Survey. The form collects data from the CMS 2567 form. It also asks for some information to help you manage the Survey. You will enter deficiencies on another page.

Click Submit to save the Survey.
